GM CLAHRC present at this year’s HSRN Symposium
Greater Manchester CLAHRC will be showcasing some of its work at this year’s Health Service Research Network (HSRN) Symposium at Manchester Central on 19-20 June. Our implementation team will be presenting some of the ground breaking work they have successfully put into practice to help patients with long term cardiovascular conditions. Cathie Stokes, Manager of the GM CLAHRC said: “The HSRN annual symposium is the premier knowledge event for health services research. This event brings together presentations and discussions from the leading edge of research and I am extremely proud to see our work in Greater Manchester playing such a prominent role.” Outlining their models of knowledge transfer and how they currently apply the theoretical models in practice, our implementation team will be presenting their projects as follows:
- Linda Savas, Knowledge Transfer Associate and Katherine Grady, Care Call Development Manager at Salford Royal Hospital, will deliver a presentation on the Impaired Glucose Tolerance (IGT) Care Call Project which recently won the QiC (Quality in Care) best type 2 diabetes prevention initiative award. Their presentation titled “An innovative approach to providing lifestyle and behaviour change to prevent type 2 Diabetes” will detail the results of the IGT Care-Call project – a six month telephone based lifestyle intervention programme for people diagnosed with Impaired Glucose Tolerance (IGT).
- Anna Betzlbacher, Improvement Manager, will be giving a presentation entitled “Translating evidence into real-life settings: A preventative service for people at risk of developing type 2 diabetes.” Her presentation will focus on the development of a successful face to face intervention offering intensive personalised lifestyle and behaviour change support and advice, to improve the lifestyle of patients with IGT.
- Katy Rothwell, Knowledge Transfer Associate, will give a presentation focusing on the implementation of a sixth month post-stroke review – a key requirement of the National Stroke Strategy. She will provide an overview of the successful development and implementation of the Greater Manchester Stroke Assessment Tool (GM SAT), an assessment tool specifically developed to assist the essential six month post stroke review.
- Mike Spence, Knowledge Transfer Associate, will do a presentation about the success of the Greater Manchester Heart Investigation Tool (GM HFIT) and how this multifaceted approach, which involves educational sessions, clinical audits, implementation of standardised coding systems and multi-professional working, has effectively improved the management of heart failure management in primary care. The presentation will encompass data from 13 practices, relating to 478 Quality Outcome Framework (QOF) documented heart failure patients. Click here for more details on this project.
- John Humphreys, Knowledge Transfer Associate, will display a poster featuring the improved methodology of the second phase of the Chronic Kidney Disease (CKD) improvement project – a project which has successfully helped 11 local GP practices identify and manage CKD to NICE guidelines.
