Impaired glucose regulation – Salford Care Call
What did we do?
We extended our impaired glucose tolerance (IGT) care call project to all GPs in Salford to build capacity and confirm that results could be reproduced. The Impaured glucose resistance (IGR) care call pathway offered a choice of services; one service was delivered entirely via telephone support and the other included a three hour group education session followed by the telephone service.
Why was it important?
The numbers of people diagnosed with type 2 diabetes continues to rise with current prevalence being over 4% in the UK, expecting to increase to over 8.5% by 2020. Risk factors associated with the development of type 2 diabetes include obesity and inactivity, particularly in those people who are diagnosed with IGR, a condition where blood glucose levels are raised above normal, but not high enough to warrant a diagnosis of type 2 diabetes.
How did we do it?
We worked with Salford Diabetes Care to refine the impaired glucose pathway, expanding the service to target all patients with IGR in Salford to prevent or delay the onset of type 2 diabetes.
