What are we trying to do?

Tameside & Glossop has recently engaged in a wide-ranging programme of work (the Care Together programme) to transform health and social care services across their region.  We are working with Tameside Metropolitan Borough Council, NHS Tameside and Glossop CCG and Tameside and Glossop Integrated Care NHS Foundation Trust to evaluate the implementation and impact of this programme in order to inform decision making on the future design of services.

 

Specifically we will evaluate the following aspects of the Care Together programme:

 

  • The process of implementing the various schemes

  • The impacts and outcomes of these schemes

  • The economic sustainability of the programme

 

Why is this important?

A fundamental aim of the ongoing Greater Manchester devolution work is to improve the health and wellbeing of the local population. To support this the Greater Manchester Health and Social Care Partnership (GMHSCP) has provided funding to each locality to deliver transformation plans to improve the health and wellbeing of their residents. Care Together is the Tameside and Glossop transformation plan and covers a wide range of schemes. The overarching aims of the programme are to: (1) improve healthy life expectancy to that of the North West average and (2) be financially sustainable within five years. This evaluation will provide our partners with valuable information about their programme, thus supporting them in their ongoing commitment to improve the health and wellbeing of their residents.

 

How are we doing it?

We are using a mixed-method approach we will deliver four inter-related work-packages:

  • Work package 1 will explore the process of implementing the various elements of the scheme

  • Work package 2 will evaluate whether or not these schemes worked

  • Work package 3 will investigate the value of these schemes in terms of impacts on local population health outcomes

  • Work package 4 will evaluate the impacts of the Care Together programme on the financial sustainability of the local economy

 

Data sources will include:

  • Semi-structured interviews with key stakeholders

  • Datasets covering hospital, social, and primary care service use and quality

  • Relevant policy and planning documents
